2) The dates you wish to visit - please check these against the available dates, given here
3) Alternative dates if your first choice is no longer available
4) The college you are studying at and how far along in the course you are
5) If there is another student you are applying with
6) One or two sentences (no more!) stating what you hope to get out of the externship
